Please, report risks, incidents, problems, and challenges on regular basis for all involved parties and as soon as you detect them by filling in this form. Risks can be reported anonymously (just do not fill out the submitted by field). Report anything you think could jeopardize the project.

A word template is available here, please send the document to WP7@icos-cities.eu. Be aware, that sending the form via email, voids the anonymity.

Please, provide a number on the scale of 1 - 4 where 1 = low, and 4 = high. This should reflect how mitigation has reduced the impact of the risk. 

This field should contain information about WHO, WHEN, WHAT was done to implement mitigation measures to reduce the risk
